It nearly happened to me this year, to the scale of about $2500 
dollars.  I got to the end of doing my taxes with Turbo Tax and I was 
getting a result of ~$3000 return.  I felt that was wrong but had no 
idea why.  If I had just trusted the results on my first, second or 
third try, I would probably have been in trouble.

\Luckily for me, I persisted and found that there was a serious 
difference between leaving one field blank and putting a '0' in it, 
which I was previously doing.  That dropped my return to ~$600, about 
where I expected it to be!

So, as others, I'm not to concerned that a mistake happened.  They 
happens to everybody!  Show where he did not honor and correct the 
mistake once it was discovered, and then there might be cause for the 
'corruption' label!  Just making the mistake in the first place is not 
enough.
