How about wanton disregard for orders. They were ordered not to launch. The location was not a combat area, it was a fricking training area for pity's sake. The pilot who launched the JDAMS was a john wayne type. He ignored 2 separate orders to checkfire.
Ordinary incident of friendly fire can almost be understandable the wrong guy at point X at the wrong time. I read the available reports from both the investigations - US and Canadian. There were many egregious errors in the Command and Control System.
