All depends on one side caring about MAD. If one side cares and the other does not, then the point is again moot. In my example, the enemies are well known. In the case of the US, the enemies may not be known (or believed to be enemies) till its to late. Going to the terrorism part of this thread, all an enemy needs to do is drive a car bomb into gas stations across the country to cause fear and panic. Go on to power plant destruction, bridges, food production, etc. and the US could be hurt greatly in no time. I could go into details of where we're vulnerable but then I WOULD get a call from the secret service. :) Nukes are great as a deterrent if you can use them and the enemy fears them. We don't live in that world anymore. We live in a world of enemies who care about destruction much more than their own lives. Enemies who think in such long terms as to not care if thousands of them die today as they'll win tomorrow. Nukes mean nothing to enemies like that.
