That's kind of the nature of private insurance, isn't it?  Any company
exists to make a profit - if you get buried in paperwork and finally
give up trying to deal with them for anything more complex than a
checkup, they don't pay out anything.  I'm consistently amazed at the
lengths Humana goes through to keep from paying for any hospital bills
for my daughter (the most recent excuse is that she's 22 years old and
ineligible, which is surprising, since she was born in 2002).  
Eventually, they always do pay, but only after a holdout.
