In our business we are the hosting provider, but I have a friend who 
does some work on the side and has his clients set themselves up with a 
hosting provider.  When it's time to do hosting, he will go sit down 
with them, bring up the hosting site (I believe he generally uses 
Crystal Tech), and will walk them through the process of setting up the 
account, billing with their credit card, etc.  This puts them in the 
driver's seat and they get to know who their site is going to be with, 
what they are going to be charged, they get the e-mails, etc.  It's his 
way of starting to cut the site loose and hand the client over to the 
hosting provider.  He stays available if they need to make additions or 
changes, of course, but if they call about the site being down or slow, 
he tells them to call the hosting provider to get help so he's not "on 
call" all the time for problems outside of his control.  As far as I 
know that type of setup has worked out well for him, and he never 
handles their credit card since the client actually puts all of the 
information into the host's site for billing and such.  Aside from him 
sitting there giving directions, he stays away from the keyboard at that 
stage.
