I have built up a component that handles header/footer and three col
layouts on the fly (as well as manipulating the number cols in a three
col layout approach (i.e hide 1 or 2 of the three) and control the
order that the left column, middle and right column appear in (i.e
l-m-r, r-m-l, r-l-m)). And handles the widths as well. It's based
heavily on Doug Bowman's (http://www.stopdesign.com/) work for
Apadtive path (you can see it in action here:
http:///www.adaptivepath.com/, e.g the homepage and the services are
the same template, but display varying numbers of columns).

Of course this is not a GUI based tool, but it has helped me save a
considerable amount of time when setting up templates. Of course
nesting other items (say two more columns within the middle one) still
requires some hand coding and patience.
