I have been using Isapi Rewrite from Helicon Tech (http://www.helicontech.com/) with great success.
There is a free version that would probably do what you are looking for. The bad news is that it is an ISAPI filter that needs to be installed in IIS, so I doubt your hosting provider would agree to install it on a shared server.
