You know, I had a problem "engaging" second and fourth that I traced to interference with, of all things, the console top plate. Are you banging your shifter into the plate on those throws? If so, give it a try without the top plate on. Just a real low cost check. It probably isn't that problem, but it only takes a minute or two to diagnose. My fix was just moving back the console a smidge.
