I think the obvious response here is not to ask why was this not fixed,
but why were you not notified of this when they were doing the sheet
metal work?  When my car was being worked on I would get calls telling
me that they found a problem or that I needed to make a decision.  For
example, they asked me if I wanted to get replacement doors or have them
take the time to patch them.  Or, if there was additional rust, they
would tell me that there was going to be more work (and thus more cost).
I paid them about $5,000 (and paid over $1,000 for parts) for the paint
and body work which was top notch, and got a great deal.  For $9,000, I
would expect exceptional work and communication.  

If nothing else, a good business person would see the rust holes in the
undercarriage and make a phone call to explain that even though it was
not discussed or part of the estimate, additional damage has been
discovered that needs to be fixed at an additional cost.  This both
informs the customer and lets you make extra profit!
