Our almost-three year old Chihuahua (male) STILL has accidents (actually, I think they are more "deliberates") in the house. I'm thinking it's consistency on our part, but he also just seems very comfortable going potty on anything soft. We use a crate (it's his bedroom, not for punishment) so we know the crate method, but with the possibility of a new baby coming, we can't have a carpet full of germs waiting her! Any suggestion that you know work?
