I agree with Elizabeth, Mare.  File a police report.  It couldn't hurt to try.  
That's what the police are there for.  To take care of the citizens of their 
town or city.  I think the driver needs to be sighted and punished for what he 
did so it does not happen again to someone else's dog.

I know I am harping on this but you really need to have a police report. The 
company is paying only because they know they are wrong. Please go to the 
police and have a report made. Then send a copy with the receipt. Just tell the 
deputy that you would like a report made! That's what they do.
