I spent 20 years doing basset hound rescue. We spayed/neutered dogs no matter the age. Older dogs got blood tests, younger dogs didn't. We had several die during the procedure for one reason or another. Nothing you did caused it, you are not at fault. Mourn her but do not blame yourself.

They were both already spayed when we got them but they were both around 5 when it was done. Anyway, sometime things just happen and you can't know the reason. But to go the way she did could be a blessing because she didn't have to suffer something traumatic at the end. That is hard to go through as well. My thoughts and prayers are with you and just know that you did absolutely nothing but love her.... no failure so don't even think that way.
