You could buy appliances from Cisco "Ironport". I heard that the largest ISP's in the world use it.
Also, You can deploy a linux server with MailScanner + Antivirus + SpamAssassin as Gateway Antispam (installed IBM - xSeries 366), it works fine for me. Currently It blocks about 90000 spams each day.
