I think it's worth clarifying that in the Whole Dog Journal's anecdote of an
individual dog ("Dodger"), and in the rest of the article, they tried to
convey that an effective behavior-mod program - when it works - does involve
owner-attention in one form or another but this has to be carefully applied
so that it does not inadvertently reinforce the problematic behavior.
They emphasized that behavior-mod is more likely to work when the dog is
young *and* the behavior is addressed very early. Medication -- when
carefully used and *combined* with behavior-mod - can sometimes be helpful,
since sometimes the med can help to break the cycle in the brain (of a
deeply-entrenched activity).
More information can be found on the web, but note that there is still no
unanimity regarding cause or cure. Each individual dog should be
professionally evaluated.
There is, IMO, no substitute for consultation with a specialist. I hope you
have access to a state-of-the-art center of vet specialists (like the Animal
Medical Center in NYC), or a university with a vet program. Good luck with
her, and do keep us posted...
