Actually, the storage format part I figured out.  I just changed the setting
to mbox and that was that (the only thing that bugged me about that is that
KMail defaults to maildir, but if you change the setting, it doesn't change
the existing mail folder to the new format, but no worries, I got around
that by just deleting ~/Mail and letting KMail recreate it in mbox format).

The problem I have is that it isn't getting the messages off the server
again.  I suspect it has stored, somewhere, the index (on the server) of the
last message it retrieved, and is only picking up messages received by the
server after that index value.  I want it to go back and get ALL the
messages on the server again.  That's what I need help with.
