You should be able to do this with GnomeMeeting. Here are a list of its
features: http://www.gnomemeeting.org/index.php?rub=2 You can also do DCC Voice
over irc. Granted most of the time you will be connecting to an irc network on
the Internet, DCC is a peer to peer protocol. Kvirc supports DCC Voice:

You could always run your own IM or IRC server. I know there are lots of them
out there. Your best bet would probably be to run a Jabber server. Try searching
