I also use Giganews.  Not the cheapest but they work reliably and are
very fast.  Their top package also offers VPN access for when you are
out and about using insecure APs.  It is also pretty handy for
watching Hulu since the endpoint is in the US.  Of course if SSL and
VPN do not matter to you, there are probably providers better suited
for you.
