) (Last but certainly not least) My biggest concern is that today we were informed that ALL records other than MDS will be thinned q 3 months. We are going to have no information on the charts that is older than 3 months, including physician progress notes, ancillary department notes, etc. The only things that will be left longer than 3 months are the required 15 months MDS, the care plan, and rehab information.
Holly, our charts are thinned to 3 mos,except mds,pt,ot and labs(6mo) and social service, We have filing cabinets on units with two years of thinned info. These are also considered active charts and accessible to everyone. Keeps the charts easier to handle. Dates of falls are put on careplan and I also will put date on MDS after printed out.
