I am also trying to work on a better B&B program.
Can you tell me when you say "we" review H&P who does the review and assessment at
your facility and what is the frequency of your B&B toileting plan?

Well Kathy you asked for it....
All admissions in our facility are assessed for continence. We review H&P,
speak to the resident and family members, howerver in saying that we also
acknowledge that we may be provided with the accurate information and may
not be.
If there is just 1 episode of incontinece the resident is put on a 3 day B&B
toilet plan. If after this 3 day period we find a number of episodes of
incontinence then we continue this plan for an additional 7 days. If we
find that upon admission the information provided was inaccurate we document
as such and continue toilet plan for dignity and contact phyisician for
further interventions.
