The "WE" consists of the charge nurse to begin with....if I note on the 5
day assessment the need to continue was not carried out by the charge nurse,
I confer with the DON bringing all information to her attention and we start
the 7 day (if not already started).
Our toilet plan is q/2hr. and prn but remember some residents require more
frequent intervals of toileting...thus the importance of an individualized
personal care plan.

Well Kathy you asked for it....
All admissions in our facility are assessed for continence. We review H&P,
speak to the resident and family members, howerver in saying that we also
acknowledge that we may be provided with the accurate information and may
not be.
If there is just 1 episode of incontinece the resident is put on a 3 day B&B
toilet plan. If after this 3 day period we find a number of episodes of
incontinence then we continue this plan for an additional 7 days. If we
find that upon admission the information provided was inaccurate we document
as such and continue toilet plan for dignity and contact phyisician for
further interventions.
