Our self-administration policy is such that residents who express a desire to give their own meds have to qualify with a cognitive assessment equaling a 14/14. Then before it can be implemented a physician's order is obtained and the MDS coordinator performs an assessment to determine if the resident understands what the medication is, what it's for and how much to use if it's a cream or something. This is re-evaluated with each assessment or when a change in mentation is noted. This also has to be very specific as to the medication, route, dose,etc. We can't just get an order that says "may have meds at bedside". On the MAR the nurses are to indicate they are checking with the resident daily to see if they're using the med and if they need to order more. This also allows the nurse to assess how well the resident knows the self-adm. process. The residents are allowed to keep creams and ointments at the bedside in this case but we haven't had anyone request anything stronger than a Tylenol; I would think they would then have to have a locked cupboard for that to be safe.
