Subject: survey question- disciplinary action used to write a defeciency?

On a recent complaint investigation the surveyors asked to see an
ex-employee's personal file. It contained a disciplinary action r/t her
residents found wet (saturated) and having dried BM on them. The employee
was reprimanded for this (she was terminated later that day due to another
issue that was not related to resident care). In the exit interview (we have
not received the written deficiencies yet) the reprimand was referred to
give us F224, F225 & F226. Has anyone ever heard of this being done?
