For all assessments other than OBRA Admission and
Significant Change/Correction, R2B date can be up to 14 days after ARD, with the
ARD being "0". VB2 (RAP Completion) can also be up to 14 days from ARD,
and VB4 up to 7 days from VB2. For an admission assessment, R2B and VB2
must be completed by day 14 of the stay, with day of admission= Day
1. VB4 (Care Plan Completion) must be completed by day 21, but also within
7 days of VB2. So if you did RAPs on day 12, then VB4 can be no later than day
19. Same for significant changes/corrections; the MDS and RAPs must be
completed within 14 days of discovering the change or error that
precipitated the assessment.
