Nurse to nurse is good IF; the nurse at the hospital that has the time, knows the patient (Isn't a temp, or "1st time I've had them") and we still need documentation for all those nice folks who audit us!

Nurse to nurse report between the SNF and the hospital is a great way to get
this info.


Within an acute care hospital, the nurses would never even think of
transferring a patient to or receiving a patient from another unit without nurse to
nurse report. How can we think it's okay to not receive report from the hospital
unit when a patient is transferred to us? But this is a soapbox for a
different day...
