A center in the area got cited for "not having the RAPS on the chart in a timely basis." Do they need to be actually on the chart by day 21? Our procedure is that they are complete by day 21 and in the care plan nurse's box along with the MDS until she completes the care plan 7 days later. She then posts the RAPS and MDS and the care plans go to our DON for signature (SB 1202 in Florida). After they are signed, they go to the chart. Do we need to change our procedure and have them on the chart earlier?
