Carol, the RAPS should be on the chart on or by day
14.  Other respondents have cited the RAI on the
regulations for completion or RAPS & care plans, so I
won't address that, but it sounds like you are asking
when they must physically be in the medical record. 
Answer: by the date the regulations say they are due. 
Remember the old saying "if it wasn't charted, it
wasn't done"?  That holds true in most situations.  If
the RAPS are not on the chart by the 14th day, there
is no way to prove they were done on
