The signature and date at R2b signifies that the MDS is complete.  The signature and date at Vb2 signifies the completion of the RAPS.  Those 2 dates can be different.  Both of the dates, R2b and Vb2 must be on or before day 14 for an admission assessment and on or before day 14 after the ARD for annual assessments.  You have 7 days to complete the care plan after the Vb2 date--which in Holly's example is after the R2b date(perfectly acceptable). Vb2 must be on or after the date in R2b.  Look at page 2-5 in the revised manual.  The second bullet says" Both the MDS completion Date (R2b) and RAPS Completion Date (Vb2) must be dated within 14 days of admission.  R2b must always be earlier than or the same as Vb2.  If R2b is dated prior to day 14, Vb2 may or may not be the same day, but can be no later than day 14." 
The next bullet shows that the care plan must be completed by 7 days after Vb2

I thought that when you signed at R2b on an admission assessment, you were signing that the RAP's and MDS were completed, and then you had 7 days to complete the care plans.  For the instance cited in the email, she stated that they were (the RAP's) completed by day 21, making these assessments after admission, in which case the RAP's would ned to be done by day 14 at the latest.  For an annual assessment you have ARD + 14 days, but not later than VB2 of the previous year (not more than 366 days.)
 
Am I missing something here, AGAIN????

The raps have to be completed by the date in Vb2.  This is the date that the RN signs and verifies that the raps have been completed.

Rap;s neet to be completed by the day the MDS is signed at R2B, and you then have another 7 days to get the care plans completed.  They might not need to be ON THE CHART by day 21, but they do need to be available to clinical staff.

RAPs, which should include your decision to careplan, are to be completed by day 14 for an Admission OBRA MDS.

RAI User’s Manual VB2:

                     Date that the RN coordinating the RAP assessment process certifies that the RAPs have been completed.  The RAP review must be completed no later than day 14 from the admission date for an Admission assessment and within 14 days of the Assessment Reference Date (A3a) for an Annual assessment, Significant Change in Status assessment, or a Significant Correction of a Prior Full assessment.  This date is considered the date of completion for the RAI (i.e., the date used to determine compliance with Federal time frames for assessment and the date that drives future due dates for when the RAI needs to be completed).                                                                                                                                   (p. 3-238)

 

The Careplans are to be completed no later than day 21.
