The only way that the PICC line placement could be considered an exclusion would be if it was considered "ambulatory surgery involing the use of an operating room." If that is NOT the case, and it is probably not, then it is not an exclusion. If not an exclusion, the facility would be liable for the expense of the procedure, the x-ray and the transportation. The only part of the procedure that would not be the facility's responsibilty would be the physician's professional component and the radiologist's professional component . My facility usually asks for the lines to be placed before admission to the SNF so that we do not have to bear the burden of the expense for the procedure and trip.

We are trying to determine if it would be beneficial to have a group come
in-house to place PICC lines. If we have a Medicare A resident that we send
out to the hospital for a PICC line, are we responsible for the procedure in
our per diem rate? How about the X-ray to verify placement? Transportation
to and from?
