I am in Ohio and state surveyors just left the building Wednesday about 4:30PM for our annual visit.  Care Plans took some very minor hits (that were unfortunately my own oversite darn it!) but we had a resident that wasn't current with pacemaker checks and we also will be cited for that.  There was also someone with a fall and inadequate follow up documentation.  According to the exit meeting, all of the issues will be very minor citations.  Whew, guess I can relax for the holidays now.  The past four years they either came the week of Christmas or the week between Christmas and New Years.  I have now decided that leaving some minor obvious errors for them to find.  Then they don't have to dig so much and will be happy that they found something.  Our staff is absolutely the best and care has not been anything that the surveyors have looked at in the 6 1/2 years I've been here!
