As far as I know any regulation would be state specific.  It is a common standard of 
care that the care plan is started upon admission with the first assessment.  In 
Washington state we need to have a working care plan in the record by day 7.  On day 
one the surveyors seem to expect a pretty good problem list and the beginning of 
interventions.
