I would code it as off unit (Inside Nursing Home/off unit) since you appear to have clearly distinct units. I think it is similar to the Section G locomotion off unit (Page 3-77) "If the facility has only one floor, locomotion off the unit is defined as how the resident moves to and from distant areas on the floor."

That's my interpretation.
