Our facility MDS Coordinators do evaluations of antipsychotics with each OBRA assessment and sign. change; it's not done on the antianxiety and antidepressants. All psychotropics are addressed in the context of the weekly Resident Safety Committee. Residents are assigned by the DON to each member the previous week so they have time to assess the last reduction attempt,etc. Meds/behaviors & treatments are discussed and the QA nurse is there to chair and it's then decided as a team what the best course of action to take would be. The assigned person documents in the nurses notes what the committee has decided to do and each person assigned is responsible for the follow-through, whether it's delegated to the charge nurse or whoever.
