On page 1-11 it does not give guidance as to what the date should be, however on page 1-24 it states that the "date should reflect the date of the assessment>". On page 3-11 it says to date it "the date they completed those sections. On page 3-27 it is talking about the face sheet which is different from the tracking sheet, but it does say that "...enter the date the form was signed.", and it goes on to say "... if the face sheet cannot be signed on the date it is completed it is appropriate to use the actual date it is signed."
