Tanya,
You will start over with a Medicare Return/Readmission assessment with the "assessment clock" starting over (ie.. 14 day, 30, 60, 90 )  You should also evaluate for significant change, which you can combine with either the R/R or 14 day assessment.  Also, remember to deduct the days used from your 100 days in calculating the last covered day.

If a resident was Medicare skilled had a 3 day qualifying stay, the
Admission, 5 day MDS was completed but prior to the 14 day assessment the
resident was readmitted to the hospital, has been there since 11/20 and is
returning today, what kind of assessment do I do?
The bed was held the entire time the resident was in the hospital.
