Weight changes for Items K3a and K3b are determined based on point-in-time comparisons of the resident's weight:  The weight in the current observation period is compared to the weight 30 days ago and to the weight 180 days ago.  Those are the rules that must be followed.  It would not be appropriate to substitute the admission weight. 

It is important to identify the preadmission weight trends in order to be able to care plan and head off continued problems whenever possible.

Please help with this question regarding calculating weight loss/gain. Our dietician has been looking back 30 to 180 days for admission weight loss percentages as on page 3-151 section a. This does at times give us a tremendous weight loss as family and previous weight gathered somewhere else are not entirely reliable. Do we have to go back 30 to 180 days or can we use just our admission weight?
