Umm, I have a problem with this one........
Been a diabetic for a while, and I can tell you that most
folks are fairly uncomfortable with a blood sugar of 500. Most folks
are barely conscious, they're drowsy, very thirsty and can't see too well
around all the blurry lines. On the other hand, driving insulin down too
quick can cause other problems. I guess I would want to know if they
called the Dr before shipping out to the ER?
In the ER, they'd be admitted to ICU w/ an insulin drip, so
maybe treating in the SNF prior to admit wouldn't be a great idea, especially if
they were using the standard Regular Humulin, rather than Lispro Humalog, which
is over & done with in 2 hours. I would want to know how often the pt.
was checked for CBS, how long they'd been diabetic, what kind of diabetic they
were (IDDM, NIDDM), if the nursing facility had one of the d*&[EMAIL PROTECTED]
"liberal dibetic diets", if the patient was A&OX3 normally, or if demented,
and if any other infection were going on before I blasted them out of the water,
but my guess is that no one was watching or the sugar wouldn't have gotten that
high.
(Now, after politely getting down off my soap box and storing
it away, I have to admit that my CBS was 462 last night after my husband and I
had a major fight, and it was 167 5 hours before the fight.) It isn't
always black and white.

Does anyone know of a group or
another listserver where nurses like myself might have a question and some
nurses could give their opinions, like here.
I have been out of the
hospital setting for 20 years ever though I keep up my CEs and read journals.
I would sometimes like to throw out a question about a nursing assessment
but I don't know if this is the place or is it?
Ex: Today I was at a nursing home
on a complaint and the DON told me that this resident's BS was 498. It was
documented.and the nurses at the nursing home were told never to
give a resident insulin before going to the hospital because
the hospital nurses and doctors couldn't assess her for hours until the
insulin wore off. The MD was called, VS taken and the resident sent to
the hospital without insulin. Has anyone ever heard of this.
This is something I would like to learn. So I would like nurse mentors for
questions too.
So if anyone can guide me if
their is a place, I would very much appreciate it. And thank you all for your
