However, in my RAP, I'd address that the area had once been a stage IV. Just playing devil's advocate - If the resident had been re-admitted to the same Skilled Nursing Facility and that facility knew it had at one time been a Stage 4 how would they stage it as compared to a facility that doesn't know what it had been in the past?
