This is from the PPS Final Rule (July 30, 1999, page 41662)

If the physician orders therapy for 10
days, the projected number of days in
section T will be 10 rather than 14;
likewise, if the physician does not order
a limited number of days, the projection
will be based on the entire two weeks,
assuming the beneficiary's continued
stay and receipt of services.

In either case, of course, you bill for only the number of days the resident actually qualified for coverage.

Looking for a regulation or standard for this scenario: 

 
      Skilled therapies projects a high rehab RUG for 5 day.
 
       Patient is discharged on 3rd or 4th day.
 
      Actual minutes don't come anywhere near projection. 

Opinions welcome as well as direction to regulation or standard.
