I have had many instances of the same where a resident is discharged home early, or leaves ama, or goes back to hospital.  The purpose of the projection is to "estimate" the # of days and minutes that the resident is anticipated to achieve (under perfect circumstances).  But as we all know,  it is not a perfect world.  We keep the 5 day as it is, using the projection.  If the resident is readmitted with no significant change, I do a "0,5" (medicare re admit.) and use the short form.  Hence this 5 day assessment only accounts for the actual days and minutes the resident accomplished.  If they are a significant change, I do a "3,5", rehab reprojects for the new conditions.  If the sig change is such where the resident is not able to perform rehab, then we do a 3,8 to indicate a sig change with an omra.
