The therapists should sign off on the information, it can be a single
representative of therapy that gathered and verified the data. It depends on
your system and set-up whether other people actually enter the data in the
computer or just provide the information and then sign off on the printed
form. Christine

Subject: SECTION T & THERAPY MINUTES/DAYS ON SECTION P

I'D LIKE INPUT FROM YOU AS TO WHO ENTERS INFORMATION ON SECTION T & SECTION
P (THERAPY DAYS/MINUTES) & WHO SIGNS OFF ON IT.

CURRENTLY, OUR FACILITY HAS THE CASE MANAGERS ENTER IT AND SIGN OFF ON IT,
EVEN THOUGH THE THERAPISTS ARE RESPONSIBLE FOR THAT INFO.

THANKS ALOT!
