I will put in the #of days & minutes; it then given to the various therapist to check and make sure what I am going to send to the state is accurate. They are to inital it w/one line through w/the correction if indicated, then they are to sign off on the section. I am not the therapist, and if the information is ever questioned or reviewed, it will be the therapist not me..
