I input and sign for the minutes in both places.  Per the August 2003
update, it was deleted that a therapist had to attest to accuracy of
minutes.  My therapy manager gives me a copy of section P (therapy
minutes) and section T and I input these minutes.  It is like anything
else that I input, ie number of meds, I am only counting the number of
medications given, there is no way I can truly attest that they were
given.  The therapies document their numbers on a grid and it matches
the minutes I am given.  I feel very comfortable doing this as I have a
very meticulous therapy manager.
