Is the reason they are taking the Synthroid
listed in the medication order? If not, why not? If the order reads Synthroid
xxmg PO QD for hypothyroidism and it is signed by the MD, then they have the
diagnosis. If it is not listed, then you can fax or call the MD for a reason for
the medication, which you need regardless of the MDS, and you can get the
diagnosis on the chart. We do that for all meds if the MD does not indicate a
reason for giving each one. We also get dx of depression and everything else
that way because they often don't list depression in the H&P but they are on
an antidepressant. After a while, most of the docs get the idea that we need a
reason for each medication when they get faxes requesting dx and clarification
for meds on all of their admits to us.
