You don’t unless you can get a diagnosis before the ARD.  I usually cover it in my care plan by stating that resident or family states has hx of hypothyroidism.

If you have a patient and the physician does not document in H&P, or anywhere that they have hypothyroidism, and they are on Synthroid, would you code Hypothyroidism on the MDS?
