We have a care conference "Sheet", where everyone signs in, including family & resident if they come.  Under the notes section, if family doesn't come, I will note:  Family did not attend although invited.  (They are all sent a letter, with the day and time, with a note that if it is not convenient, that they can call and request another time.)
 
That covers us, and indicates the level of involvement of family, legal guardians, etc.

Question for the group……..I am not seeing my answer in the SOM or RAI. Is there a regulation regarding specific time frames where we must show that resident / representative has been involved with the IDT and the care planning process?  Example… care conference?   

I am not sure if this is a state requirement for us or CMS……..needing documentation.
