I would go one step further. Unless you have an order for treatment (Generic eval and treat is questionable at best), you can't project. I encourage everyone to get a second order that gives specifics of the therapy to be delivered.
If the therapist does the eval, and then writes a recommendation for treatment in days and minutes, I think it would be ok for the MD to then order treatment as recommended by the PT, but the three events (order to eval, evaluation and recommendation, order for treatment) need to take place in that order.
