A standing policy to use the grace days for every
5-day is a bad idea. The safest way to appraoch this is to use the grace days
when doing so causes the MDS to more accurately reflect the care being delivered
to the resident. For example, if you have an order for therapy that
will qualify for High or Very Hight, but the resident was admitted on
Friday and the first therapy is not delivered until Monday, setting the ARD to
day 5 (Tuesday) would show only 2 days of therapy actually delivered. Using 3
grace days moves the ARD to the following Friday and now you show 5 days of
therapy. This is more indicative of the actual level of care being
given.
