restraint question

Dear Group: We had a surveyor find a resident with a belt on a wheelchair yesterday.  PT did not get a doctor order yet. They were trying it out first to see if she didn't slip. The surveyor sited us for a restraint. And then looked at me for my documention.. Of course I had none, that's because PT did not tell me they put it on. her.  Now where do I start? I am a new MDS nurse. Can this even be corrected?
