Does you
facility have a restraint policy, and was it followed? We have no
restraints in our building at this time, we can only apply a restraint after an
IDT meeting, and a therapist would never put on a restraint without the nursing
dept knowing. It is too dangerous a situation. ie release schedule etc.You
should not be held responsible, but your facility will likely will get cited.
The fix is realtively easy though, no more restraints applied without a meeting.
Documentation etc. Can the therapist throw herself/himself on the mercy of the
surveyor. We are in survey too right now. GOOD LUCK.

We had a surveyor find
a resident with a belt on a wheelchair yesterday. PT did not get a
doctor order yet. They were trying it out first to see if she didn't slip. The
surveyor sited us for a restraint. And then looked at me for my documention..
Of course I had none, that's because PT did not tell me they put it
on. her.
Now where do I
start? I am a new MDS nurse. Can this even be corrected?
