As far as the surveyor, fall upon the sword and get something in place now. Look in therapy's plan (which should be signed by the MD) as to whether or not a restraint was in the plan. We have a policy that allows nursing to apply restraints for 24 hrs without an order if the rt. is at risk for personal injury. That wouldn't cover this. Communicate with Pt the need for an order with or without a trail AND for improved communications.
