As far as the surveyor, fall upon the sword and get something in place now.
Look in therapy's plan (which should be signed by the MD) as to whether or
not a restraint was in the plan. 
We have a policy that allows nursing to apply restraints for 24 hrs without
an order if the rt. is at risk for personal injury. That wouldn't cover
this.
Communicate with Pt the need for an order with or without a trail AND for
improved communications.

 Dear Group:
We had a surveyor find a resident with a belt on a wheelchair yesterday.  PT
did not get a doctor order yet. They were trying it out first to see if she
didn't slip. The surveyor sited us for a restraint. And then looked at me
for my documention.. Of course I had none, that's because PT did not tell me
they put it on. her.
 Now where do I start? I am a new MDS nurse. Can this even be corrected?
